CONSENT FORM
For young people under 18 yrs this form must be completed by parent(s), guardian(s) or responsible adult
Skateboarding, BMX, inline skating and scooting are hazardous activities! All members are strongly recommended to wear full protective clothing – helmet, kneepads, wrist guards and elbow pads. It is COMPULSORY for Helmets to be worn by all those under 18 years and any age if using the Vert.
Skaterham will supervise the park with qualified First Aiders and volunteers but it cannot accept any responsibility for accidents which may occur as a result of the activities of skateboarding, BMX(or other bike riding), in-line skating, quad skating and scooting.
All new members to public sessions are required to undertake a brief induction to include FIRE SAFETY, RAMP ETIQUETTE and SAFE USEAGE. Please also read our ACCIDENT ANAYLSIS display by the reception.
Please inform reception on your first visit, we are more than happy to give a brief tour and guidance on ramp etiquette. Just like crossing a road, all users should check before setting off and look both ways when crossing any ramp.
I understand and accept that I am/my child is using the park and ramps entirely at my/her/his own risk and, when less than 18 years old, with a parent’s/guardian’s knowledge. In accepting this risk I agree to waive any legal entitlement to claim against Skaterham its staff or volunteers, or its landlords for injury or loss or damage to personal property.
